Does Tri-State offer any financial assistance to patients?
Financial Assistance is available to people and families who meet certain income requirements. You may qualify for free care or reduced-price care based on your family size and income, even if you have health insurance. For more information, refer to the Financial Assistance Policy under Policies and Regulations.

What forms do I have to fill out in order to become a patient at Tri-State?
In order to become a patient with one of our on-campus providers, you will be asked to fill out the New Patient Form. That form is located under Patient Forms.
