Does Tri-State offer any financial assistance to patients?

Financial Assistance is available to people and families who meet certain income requirements. You may qualify for free care or reduced-price care based on your family size and income, even if you have health insurance. What forms do I have to fill out in order to become a patient at Tri-State?

In order to become a patient with one of our on-campus providers, you will be asked to fill out the New Patient Form. That form is located under Patient Forms.
