My Health Portal is a secure online website that gives patients convenient 24-hour access to personal health information from anywhere with an Internet connection. Using a secure username and password, patients can view health information such as:
The Tri-State Kidney & Hypertension Clinic treats patients who are diagnosed with kidney diseases and/or hypertension as well as patients requiring renal replacement therapy, which includes dialysis and renal transplant patients.
You may be referred to a nephrology specialist for reasons such as:
Acute renal failure, or a sudden loss of kidney function
Chronic kidney disease, or declining renal function, usually with an continuous rise in creatinine
Hematuria, or the presence of blood in your urine
Proteinuria, or the loss of protein, especially albumin, in your urine
Kidney stones, usually only recurrent stone formers
Chronic or recurrent urinary tract infections
Hypertension that has failed to respond to multiple forms of anti-hypertensive medication, or that could have a secondary cause
Electrolyte disorders, or an acid/base imbalance
Admission to Tri-State Kidney & Hypertension is for patients 18 years of age and older through referral only. Once your primary care provider has referred you to Tri-State Kidney & Hypertension, a New Patient packet will be mailed to you for completion.
Please return the completed forms in the postage-paid return envelope as soon as possible so we can complete your admission process quickly.
Please bring your current medications in their original bottles with you to your first appointment.

Meghana Awad, MD completed medical school at NDMVP Samaj’s Medical College, Pune University, in Nasik, India. She then completed an infectious disease fellowship, also in Mumbai, in addition to an internal medicine residency at Lincoln Medical and Mental Health Center, Bronx, NY, and nephrology fellowship at Virginia Commonwealth University of Health Sciences in Richmond, VA. Dr. Awad is certified by the American Board of Internal Medicine (top ten percentile) and is a member of the American Society of Nephrology. Dr. Awad stood first among a class of 120 in her second year of medical school with honors in Pharmacology.
