Medical/Dental/Vision Insurance

Tri-State Memorial Hospital & Medical Campus offers Medical/Dental/Vision insurance coverage for all full-time regular employees at the hospital’s expense. The program is administered by Premera BlueCross of Washington. Medical deductible is $500 per year for the PPO Plan or $2,000/$4,000 for the HDHP, which is partnered with an HSA. Dental deductible is $25 per year. Dependent vision coverage available; details of coverage is explained in the Delta and LifeMap summaries, available through the Human Resources Department.

Available to full-time employees on the first of the month following hire date.

Who Pays?

Employee's expense paid in full by Tri-State Memorial Hospital & Medical Campus. Pre-tax dependent Medical, Dental, and Vision coverage available at employee’s expense.

Paid-Time Off (PTO), Extended Illness Bank (EIB)

PTO is to be used for vacation, holiday, and short term illness and/or injury. Annual accumulation includes 6 holidays, 2 personal days, 10 to 20 vacation days (accumulation is per hour paid and varies based on years of service), and 6 sick days for illness/injury of 16 hours or less. EIB to be used for long term illness after 24 hours of continuous absence. Annual accumulation of 6 days per year (48 hours) for a full-time employee. Max accumulation of 100 hours.

Short-Term Disability (STD)

STD benefit, administered by LifeMap, is available beginning 15th day of disability due to sickness or accidental injury and pays a weekly amount of 60% payable up to 11 weeks with a physician’s release from work. STD is used in conjunction with FMLA.

Long-Term Disability (LTD)

LTD benefit, administered by LifeMap, is available after 90 days of disability. Waiting period may be served with total or partial disability or a combination of both. Benefit pays 60% of your monthly pre-disability earnings.

Retirement Plan

All employees may elect to contribute a percentage of their annual compensation to our Lincoln Alliance Retirement Program, not to exceed limits of IRS Sections 403(B). Employer contributions are percentages based on years of service. The contribution grid is outlined in the retirement plan booklet available in the Human Resources Department.

Eligibility

Employee must complete 12 months of service, 1,000 hours, and attains age 18 to be eligible for Employer Contributions. Contributions are based on length of employment.

Life Insurance

LifeMap coverage for full-time employees at hospital’s expense. Administered by LifeMap Insurance Company. Basic Life Insurance = $50,000 Accidental Death and Dismemberment = $50,000 Spouse Life Insurance = $10,000 Dependent Life Insurance = $5,000.

Flexible Spending Account (FSA)

Health FSA’s are an employer established benefit with employee elections made prior to the plan year start date. This plan provides employees with a tax-free way to save for out-of-pocket qualified medical and dependent care expenses.

Available to full-time employees after successful completion of introductory competency assessment period. FSA – applicable with PPO health plan. LPFSA – applicable with HSA for medical, vision, and dental expenses. LPFSA – applicable with HSA for dependent care expenses.

Bereavement Leave

Tri-State offers full-time employees Bereavement Leave, up to 24 hours in a 10 day period in the event of death in the immediate family, payable only for scheduled work days. Immediate family is defined as spouse, parent, parent-in-law, child, sister, brother, grandparent, or grandchild. Additional time for bereavement may be arranged through your Department Manager as PTO or non-paid time.

Tuition Assistance

Tuition assistance is available to employees who average 500 hours over a consecutive six month period for job related training, college degrees, either academic or voc-tech. Eligible employees will be reimbursed 50% of their “out-of-pocket” tuition cost, not to exceed $1,500 per calendar year. Tuition Assistance reimbursement does not include book costs, miscellaneous fees, and/or late fees.

Shift Differentials

Incentive pay differentials are paid to employees working evening and night shifts. Evening Differential = $2.00 per hour Night Differential = $3.25 per hour

Standby/On-Call Call Back Pay

Standby-On-Call pay is $4.00 per hour. Call back, except for low census, is paid at 1.5 times the hourly rate.
